Three university-affiliated hospitals. Between August 1992 and June 1995, 528 patients underwent laparoscopic surgery and were randomly assigned to laparoscopic colectomy, laparoscopic appendectomy, and laparoscopic cholecystectomy. The operating time and complications were recorded. Patients with complications were studied for peritoneal hemorrhage, defined as a progressive increase in intra-abdominal volume necessitating surgical control. In patients who had peritoneal hemorrhage, the extent and severity of bleeding were evaluated. A total of 528 laparoscopic procedures were performed with a mean operating time of 63.2 +/- 11.1 minutes (range, 15 to 180 minutes). The complication rate was 3.1% (15 of 528 cases). Peritoneal hemorrhage occurred in 3 patients (0.55% of all cases) and in 5 cases (0.9% of all cases) only on postoperative day 1. These patients were treated by the placement of abdominal drainage tubes. No relapses occurred. All hemorrhages resolved spontaneously. The mean age of patients with peritoneal hemorrhage was 45.5 years. None of the patients had a history of anticoagulant therapy or antiplatelet therapy. All patients had concomitant diseases; none was a smoker, and none was a heavy drinker. Peritoneal hemorrhage is a rare complication of laparoscopic surgery. In patients without predisposing factors, the bleeding is mild and self-limiting.
